How Supplied Packaged in dry state in single-size individual, sterlised, pouches, pre-loaded inside a polymide sheath acting as a guide for insertion. (2 or 10/carton). See manufacture's full data. |

Dosing See Notes 1. See full product instructions before any use. Anaesthetise punctum and insert a single Form Fit plug (in dry state via its pre-loaded inserter) into the lacrimal canal via the puncta, where it will swell to fill the vertical canaliculus. |

Indications Dry Eye. Long-term closure of punctum in cases of Aqueous Deficient Dry Eye. See manufacture's full data. |

Contra-Indications/Cautions Hypersensitivity to product/class/component. Pregnancy/lactation - EDDB. Caution with existing ocular surface Inflammation. Existing implant/punctum plugs in place. For very small puncta dilation may be required. Store at room temperature. See manufacture's full data. ECP/HCP to read and understand all accompanying product information before first use. |

Adverse Reactions In cases of canaliculitis can be flushed/irrigated out into the nasal passage. See manufacture's full data. If vision changes or irritation occurs, persists or increases, pt to discontinue use and consult ECP/HCP. |
