This is a continuation of a Linkedin thread originating from Nas Hinglotwala, who commentated on the poor ratio of MF prescribing:
Time, or rather the lack of it is the CL the enemy:
1. I have conducted clinical trials on three different MF lenses, all showed assessment at 3 hours (with patient out and about) significantly reflects 'success' rather than 20 minutes or certainly 5 minutes!
2. Insufficient chair time at first visit, clinical exam, getting to understand patient, setting expectations, lens selection should not/cannot be done in under 25 minutes, the result often is a negative decision by practitioner and/or patient when finding a positive one could have been found if only ....
3. Trying to time save in practices where the on-eye assessment is carried out during the teaching appointment!

MFs definitely need plenty of time to settle and you can’t do it properly in a 20-25min.
What we try to do now is for the Px to have the lenses applied by me, then send them out for a walk about while I see my next Px.
Then the6 come back for fit & va to be checked.
This is obviously much less than the time mentioned above and it would be rather difficult to do as many fits as we currently do if we left the Px have the lenses in for 3hrs on first insertion.
We normally advise that at the 1-2 week check that we refine the Rx and they are given a short leaflet explaining adaption to MF cls

Reading the article in Optometry Today entitled "Setting Realistic Expectations"

I entirely agreed. However it did bring to mind many of the patient's expectations are set long before they are seen by the prescribing CL Practitioner, it is unfortunate that through a combination of lack of knowledge and commercial pressures some practice staff paint a picture that MF CLs are a quick, easy and cheap panacea for every patient with any visual need. I'm sure this doesn't happen in Simon Donne's practice, however I'm sure there is many a CL Practitioner who wishes they could start with a clean sheet when discussing MF CLs, rather than having to erase the patients misconceptions gathered from other sources.
