I would like to challenge the GOC to justify their rules on why a cl rx has to be issued for a private service? Once the rx leaves the practice I have no control over duplication, over wear or substitution yet Im expected to take responsibility.

Currently substitution is legal as all cos including RGP & Hard have the same EN number.

As they do not surrender the rx the patient can duplicate as many as they want while the rx is in date giving the many years supply.

Children are able to over wear lenses with no restrictions and no guidance once they leave the practice.

Solutions can be substituted which make the aftercare a joke if you've checked for solution intolerance with a specific product.

This is becoming more of a problem with unregulated online sales and a GOC who appears to have no power to control the online supply of contact lenses and glasses online to anyone including children & venerable groups.

If we have to issue a rx surely it should be treated the same as a medical rx. It should state number, wear times, and should be exchanged for the product same as at a pharmacy. Solutions and eye drops should be sepcified if required. Any deviation from the exact rx should invalidate the rx.

The current system is open to abuse.

Anyone know where to start? Can the GOC, ABDO bring the subject up at their next board meeting? Who wants to support me.

I entirely understand your real and legitimate concerns, the current situation is a mess. However for the foreseeable future I can't see legislation being brought in to rectify the situation. A CL Rx has to provide a minimum degree of information, but no reason why additional instructions as you propose can't be added.

In an age where anyone can obtain any medication/chemical product they wish, at any time, from the internet, the street corner or the oik in the pub I think the only course of action that gives me any reasonable reassurance is to only see CL patients who are an active paying member of a care plan, where they obtain all their eye-care and products from the practice.

I recall every 6 months and state on their Rx; substitution and failure to keep after-care visits invalidates Rx along with your other proposed Rx statements.

I simply do not allow myself to be held hostage to the the minefield of any other way, does everyone like that approach - no, but we all know where we stand from the beginning, prospective patients are told they will join a care plan, can't buy lenses elsewhere and have to pay the initial exam fee on booking the appointment!

Bending over backwards while putting myself at increased risk, doesn't seem to actually make the patients any happier somehow, nor me, nor the practice staff.

Completely understand and agree with your frustration.
